Why Some Clinics Don't Bulk Bill

Why Some Clinics Don\'t offer Bulk Bill

One of the most common concerns for patients seeking healthcare is whether the doctor bulk-bills them. It has become harder for the average Australian to find a fully bulk-billed appointment over the last decade. The reason behind this change is that many people are wondering why their local clinic started charging out-of-pocket fees.

To understand the main reason for private billing, one needs first to understand what happens behind the reception desk. General practices in Australia are similar to those in any other business, except for paying taxes and following other government regulations. Even though patients get government subsidies through Medicare, the clinics themselves are responsible for their operational costs, staff wages, and other expenses related to maintaining the facilities.

If you want to understand the difference between bulk billing and how your family's budget can be affected, it is a great idea to review our Bulk Billing & Medicare Guide. This essential document provides all the information you need about the system. That said, when the government rebate does not cover the actual cost of providing high-quality care, doctors have to make the difficult decision of charging patients out of pocket. By learning about these financial models, patients will be better able to make wise choices and understand what their healthcare really costs.

Reasons Related to the Practice

A single reason rarely drives the choice to stop bulk billing. For a medical practice to survive, it needs to generate income that covers its operational overheads and even allows it to reinvest. The point is that the fee you pay (or the rebate the doctor claims) when you visit the doctor is not just for the doctor only, but to sustain the entire ecosystem of the practice.

Running a modern medical facility is incredibly resource-intensive. A significant portion of a clinic's revenue goes toward administrative staff, practice nurses, commercial rent, medical supplies, insurance premiums, and advanced clinical software. Over the years, the cost of these operational necessities has risen sharply in line with standard inflation.

That said, the Medicare rebate, which is the amount the government pays a doctor in case of a bulk-billed consultation, has not gone up as much. There have been times when the government put a stop to raising doctors' Medicare rebates so that the payment they receive does not change, even though their business costs keep going up. Doctors asking patients to pay for part of their care is actually the biggest reason.

MixedBilling

Instead of going private as a whole, some clinics go for a mixed-billing approach. This allows a practice to remain financially viable while standing by its commitment to look after those in the community who are in the greatest need.

The clinic will charge privately for adults earning an average wage while continuing to bulk-bill those who qualify for concessions, such as children under 16, older people receiving the Age Pension, or Health Care Card holders. If you belong to one of these groups, you need to know your exact rights. We also suggest that you check the concession card bulk billing guide for more details on the eligibility criteria. Similarly, you may want to look at our bulk billing for Australian families section for specific information on how we support families financially.

When private fees are paid at a mixed billing clinic, a government rebate is still available to the patient. The patient can first pay the full amount of the bill and get the subsidised portion back from the government. Once you know about the difference between Medicare and private billing paradigms, you can easily navigate the process.

How 13CURE Manages Billing for Urgent Care

Being a deputised medical service with a focus on after-hours care comes with its share of very different operational challenges and requirements. The doctor access at 13CURE after hours is based on a highly specialised model, both business-wise and billing-wise.

13CURE after-hours telehealth support is only for urgent cases; the regular GP should see non-urgent cases during the day. To maintain your health, especially if you have a chronic condition, long-term continuity of care is central. Thus, we strongly encourage you to consult your regular GP for non-urgent cases during the day.

Making care available to patients with urgent conditions overnight is quite costly. Given our commitment to ensuring the After-Hours Urgent Care service remains sustainable. It provides immediate access to medical care in the case of sudden, acute illness. Our pricing is fixed and well-structured.

In certain limited circumstances, bulk billing may be applied only to patients from partner practices deputised with 13CURE, infants under 12 months, homeless individuals, and those from a disaster area. All other patients are charged privately. Additionally, while our after-hours clinic only sees patients with urgent conditions, we do see patients privately for Medical Certificates and related documentation. If you require our services tonight, you can easily book an after-hours doctor through our digital portal.

Geographic Limitations and Compliance

Our key goal is to deliver outstanding urgent virtual health services to the communities where we operate. At present, our work is concentrated mainly in selected areas of New South Wales and Queensland.

If you happen to be in Victoria and are looking for an After-Hours GP in Melbourne or assistance in South Australia, we do not currently offer services in those areas. Still, we plan to operate there in the future and will keep you updated. This will also help us avoid any compliance issues.

Healthcare Affordability and Implications

This change from universal bulk billing has no doubt left patients facing the financial side of things more than before. On the other hand, the healthcare system in Australia has several safeguards to ensure that no one is financially burdened when seeking the necessary medical care.

The national safety net is one of the most important such mechanisms. If you or your family make frequent visits to a private billing clinic, your total out-of-pocket expenses (gap fees) will increase. When you reach a certain yearly limit, the government assumes responsibility, and it will pay a higher rebate for all subsequent out-of-hospital services until the end of that calendar year. Our explanation of the Medicare safety net will show you how it protects your budget.

Healthdirect Australia is an Australian Government website for anyone looking for additional independent help with the costs of primary care and finding services that fit their financial situation. In the end, knowing what really goes into running a clinic gives you the power to decide on your long-term healthcare plan.

Frequently Asked Questions

Yes. GPs in Australia are small business owners. They are allowed by law to charge whatever price they think is fair for their time and expertise. The Medicare rebate is only the portion of the fee that the government contributes; it is not a limit on the total charge a doctor can make.

Each mixed-billing clinic operates under its own policy. Most of them also make their fees public either through their website or at the front desk. It's best to always confirm with the receptionist at booking whether your status (e.g., a child under 16 or a Health Care Card holder) qualifies you for a bulk-billed appointment.

If your clinic lodges the claim electronically on your behalf immediately after your appointment, and your current bank details are registered with Services Australia, your rebate is usually deposited directly into your bank account within 1 to 2 business days. If you prefer to manage it yourself, you can read our guide on claiming Medicare rebates online.

Having a Pensioner Concession Card in your hands does not automatically make you eligible for bulk billing with 13CURE. We are very strict about bulk billing only for infants under 1 year, homeless persons, persons in disaster areas, and patients of the deputised partner practices. Pensioners who do not meet these exact criteria will be charged privately.
