Prescriptions & Referrals in Bulk Billing Consults

The financial benefits of consulting with a doctor without paying out of pocket extend to the clinical outcomes of your consultation as well. Patients often wonder whether administrative and clinical tasks, such as prescriptions or referral letters to specialists, qualify for the same government rebate.

Knowing how the system handles these documents will help you avoid being surprised by additional costs. To understand how these clinical results relate to the national healthcare program, you may want to revisit our introductory Bulk Billing & Medicare Guide.

Prescribing letters for bulk-billing referrals and the issuance of necessary medication can be considered deliverables from your no-gap consultation, but bear in mind that such documents are issued only in very defined clinical settings for the patient's safety and are Medicare compliant.

Referral Letter Requirement

GP Referral is a procedural step that enables a patient to access a specialist under the Australian public healthcare system. GPs can treat a wide range of illnesses, but in some cases, the patient needs to be seen by a specialist.

The referral system during a bulk-billed appointment is explained below:

  • Initial Diagnosis: The GP needs to examine you and find out the nature of your problem before they can offer you a referral to a specialist. Doctors do not give referral letters on demand; they only do so based on clinical findings.
  • Medicare Rebate: The referral letter is the document required for Medicare reimbursement when you visit a specialist. It must be dated before the date of consultation with the specialist.
  • Referral Validity: While referrals from a GP to a specialist typically last for 12 months, referrals from one specialist to another are valid for only 3 months.
  • #Bulk Billing: Letter Composition# If, during the bulk billed consultation, your GP considers a referral is necessary, the preparation and sending of that letter are already part of the Medicare rebate for the consultation, no extra charge.

For a chronic patient using a virtual care service, one way to differentiate between online visits and in-person doctor visits is to read about telehealth vs. in-person urgent care.

Prescribing Philosophy

As gatekeepers of pharmaceuticals, doctors have an enormous responsibility. This is why every GP script is tightly policy-governed in Australia to mitigate drug abuse and keep patients safe.

Doctors' ethical obligations impose many controls on their freedom to prescribe medicines, whether the patient is paying a fee or being bulk-billed:

  • Prescriptions not automatic: Even consulting a doctor will not result in a prescription. It has to be medically warranted.
  • Choice of drug: The active ingredient of the medicine is what doctors usually prescribe, rather than brand names. This allows pharmacists to offer generic drug alternatives as covered by the Pharmaceutical Benefits Scheme (PBS).
  • High-risk substances: Medications such as Schedule 8 (S8) controlled drugs (for example, high-strength painkillers or certain tranquillisers) are scarcely prescribed in first-time consultations or via telehealth for safety and compliance reasons.

Knowing about medications after hours, how deputised services operate will influence your expectations about treatment, should you require urgent medicine outside of normal hours.

Digital E-Prescriptions and Telehealth Referrals

The digitisation of the Australian healthcare industry has completely changed the way patients receive medical care. Digital ones are replacing paper documents in a second.

Through virtual consultations, your outputs are sent securely, namely:

  • E-Prescriptions: Rather than receiving a hard copy of the script, the doctor will send a protected token (often a QR code via SMS or email) straight to your phone. Just take this token with you when going to the pharmacy for dispensing.
  • Going Digital: If the visiting doctor is not technologically equipped, you may refer to our help document on how home doctors can prescribe securely.
  • Telehealth Referrals: GPs can send referral letters to your specialist via telehealth by using encrypted medical software, eliminating the need to carry the document physically.

Patients seeking information about Australia's electronic prescribing rollout are encouraged to visit the Australian Digital Health Agency.

Urgent Script Renewals with 13CURE

Your regular daytime clinic is not an option if you suddenly run out of medicine at night. 13CURE is here to help you with such unexpected, non-emergency medical situations; we even operate from 11 PM to 7 AM daily.

Our focus on urgent care defines the way we handle prescribing and referrals:

  • Short-Term Script: Upon clinical safety and need, the after-hours physician may issue a short-term script to cover you until the following day, when regular clinic hours are available.
  • Billing Structure: Our target billing model is very selective. Bulk billing applies only to infants under 1 year old, homeless individuals, residents of disaster-affected areas, and patients from our deputised partner practices. All other patients utilising our overnight service will receive a private bill.
  • Documents: We can see you privately during our shift for Medical Certificates and related documentation. Nonetheless, for ongoing referrals, chronic disease management, and long-term script renewals, a visit to your regular GP is a must.

Service Locations

The urgent telehealth network availability in the areas is geographically focused on New South Wales and Queensland.

Frequently Asked Questions

Got a question not listed here? Contact us and we'll be happy to answer your query!

No. Bulk billing is for doctors only. The pharmacy will charge you for the medication itself, though the cost might be substantially reduced by the Pharmaceutical Benefits Scheme (PBS) for certain medicines.

No. As long as the doctor is bulk-billing your consultation, making multiple necessary scripts is a part of the service. Charging a "script fee" on top of a bulk-billed Medicare claim is legally not possible for a doctor.

Missing the token via SMS for the smartphone? The physician can simply email you the e-script so that you can run it in person to your pharmacist.
